Physical Packets and UIL Forms

Physical Packets and related forms

Guyer, Denton ISD and The UIL value the health and safety of all student-athletes. We take many measures to protect our students and student-athletes. One component is a physical and other associated forms and questionnaires.

Starting in the fall of 2016 (2016 school year) we will move to ELECTRONIC FORMS. Use this link: www.dentonisd.rankonesport.com to complete the electronic forms. Additionally, Here is a printed copy of the forms:
For all returning athletes: All athletes must complete all the physical packet forms.

All athletes who are:
  1. New to the district (Denton ISD)
  2. Freshman (9th grade)
  3. Juniors (11th grade)
must have a physical completed by a physician or other qualified health care provider (HCP), as well as all physical packet forms.

Planning for College as a Student Athlete

Many student-athletes prepare to play sports at the college level their entire lives; knowing the process to play after high school is key. 
 
The NCAA has a website designed to help guide parents, players, schools, counselors and coaches through the process. 
 
As noted on the NCAA website, "Participating in college sports provides opportunities to learn, compete and succeed. Student-athletes receive top-notch academic support, quality medical care and regular access to outstanding coaching, facilities and equipment. Student-athletes as a group graduate at higher rates than their peers in the general student body and feel better prepared for life after college."

Thank you to all the parents and the area schools who participated in the 2015 Winter Classic.  The tournament was a huge success!
